SUSAN’S DOG SITTING is authorized to seek any emergency veterinarian assistance needed during visits, at the cost of the client, from any veterinarian as chosen by the sitter. However, the company is not responsible for the health/well being of the animal. I give my full authorization to use my credit card for these purposes. I further understand that should veterinary care be required, no guarantee of successful treatment is made and I agree not to hold SUSAN’S DOG SITTING or the attending veterinarian responsible.
I) Pet Owner is responsible for supplying the necessary, safe equipment/supplies needed for care of their pet(s), including but not limited to a sturdy, well-fit harness (halter, collar, etc…) for walks or in case of emergencies, firmly affixed vaccination tags, a lead rope or leash, wee-wee pads, food, medicines. Pet Owner authorizes any purchases necessary for the satisfactory performance of duties. Pet Owner agrees to be responsible for the payment of such items, as well as service fees for obtaining items, and will reimburse SUSAN’S DOG SITTING at pick-up for all purchases made.
I) It is suggested that arrangements be made with someone to evacuate your pets in case of a disaster or weather related event (HURRICANE SEASON)/crisis/”Code Red”. SUSAN’S DOG SITTING will definitely try to see to your pets safety/care should such events occur, but cannot guarantee it.
I) I agree to pay SUSAN’S DOG SITTING the boarding fee of $30.00 per night for dogs or $50.00 per night for 2 dogs from same home as applicable. The applicable rate will be doubled, on a per day basis, for each extra day the pet is boarded, if the pet is not retrieved at the end of the agreed-upon boarding time period stated herein. I further agree to pay all costs and charges for special services requested. At time of admission, all pets must be free of fleas and ticks. I agree to be financially responsible for any required treatment for fleas/ticks and/or required bathing, if determined necessary by SUSAN’S DOG SITTING. Baths $20, Frontline $20.
I) I understand that if my pet should bite a person, my pet will be quarantined in accordance with state and local laws. Daily boarding fees may apply.
I) Future Services: I authorize this contract to be valid approval for services so as to permit SUSAN’S DOG SITTING to accept all future telephone, online, mail or email reservations without additional signed contracts or written authorizations.
I) SUSAN’S DOG SITTING reserves the right to terminate this contract at any time if the Pet Sitter, in his/her sole discretion determines that Owner’s pet poses a danger to the health or safety of itself, other pets, other people, or the Pet Sitter. If concerns prohibit the Pet Sitter from caring for the pet, the Owner authorizes the pet to be placed in a kennel (or previously arranged locale), with all charges (including but not limited to transportation, kenneling, tranquilizing, treating, accessing, and liability) to be the responsibility of the Owner.
I) SUSAN’S DOG SITTING agrees to provide services stated in this agreement in a reliable, caring and trustworthy manner. In consideration of the services as an express condition thereof, the client expressly waives and relinquishes any and all claims against the company and its employees, except those arising from negligence. Claims of negligence that involve a hired Independent Contractor, hired by SUSAN’S DOG SITTING, will be the responsibility of the Independent Contractor and the company they represent. All hired Independent Contractors are required to carry liability insurance with optional coverage or bonding through a reputable company.
I) Client agrees to notify SUSAN’S DOG SITTING of any concerns within 24 hours of return.
I) This agreement is valid from the date signed, and replaces any prior Legal Considerations agreements. Client agrees to any future SUSAN’S DOG SITTING term changes relayed verbally to the client, mailed or emailed in writing to the client, or posted on our website under the heading Terms .
I) The owner states that he/she has read this agreement in its entirety and fully understands and
accepts its terms and conditions.
I) I, the owner or authorized agent of the admitted pet, hereby authorize SUSAN’S DOG SITTING to board the above described pet on their premises at XXXX NE XX Street, Miami, FL 33138 and hold SUSAN’S DOG SITTING blameless for any sickness, injury, death, or loss of this pet while boarding. I understand that natural disasters and other regional emergencies may occur while my pet is being boarded. SUSAN’S DOG SITTING will use reasonable efforts to ensure the safety and security of all pets in its care; therefore SUSAN’S DOG SITTING cannot guarantee that the pet will remain in the facility during such emergencies, if it is deemed necessary to relocate them for safety reasons.
I confirm that all the information I have provided about my pet is accurate. I understand SUSAN’S DOG SITTING reserves the right to deny admittance of my pet for any reason at any time.
Any pet left for 14 days past the scheduled pick-up date will be deemed abandoned. SUSAN’S DOG SITTING will make all reasonable attempts to contact the owner. If, after all reasonable attempts have been made with no response, the animal will become the property of SUSAN’S DOG SITTING, and as such, SUSAN’S DOG SITTING will have the authority to do with that animal as SUSAN’S DOG SITTING sees fit. I understand this action will not, however, relieve me from paying all charges for services rendered and all legal and/or costs incurred with collection for services.
